Drug interactions

Moderate drug interactions

Moderate:

anti-thymocyte globulin (rabbit) roflumilast

Applies to: Thymoglobulin (anti-thymocyte globulin (rabbit)) and roflumilast

Using roflumilast together with anti-thymocyte globulin (rabbit) may have additive effects on the immune system, possibly increasing the risk of infections or immunologic disorders. You may not be able to use roflumilast if you have a serious infection, cancer, or an immune-based disorder such as HIV infection, multiple sclerosis, or lupus. Talk to your doctor if you have any questions or concerns. Your doctor may already be aware of the risks, but has determined that this is the best course of treatment for you and has taken appropriate precautions and is monitoring you closely for any potential complications. Contact your doctor immediately if you develop signs and symptoms of infection such as fever, chills, diarrhea, sore throat, muscle aches, shortness of breath, blood in phlegm, weight loss, red or inflamed skin, body sores, and pain or burning during urination. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Food/lifestyle interactions

Minor food/lifestyle interactions

Minor:

roflumilast Food/Lifestyle

Applies to: roflumilast

Information for this minor interaction is available on the professional version.

Disease interactions

8 potential interactions Applies to: 8 conditions, including Depression, Anxiety and Stress, Liver Disease, Infection, and Immunodeficiency.

Anxiety and Stress

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Anxiety and Stress.

The use of roflumilast is associated with an increase in psychiatric adverse reactions. Instances of suicidal ideation and behavior, including completed suicide, have been observed in clinical trials. Before using roflumilast in patients with a history of depression and/or suicidal thoughts or behavior, prescribers should carefully weigh the risks and benefits of treatment in all patients. Close monitoring for the emergence or worsening of insomnia, anxiety, depression, suicidal thoughts or other mood changes is recommended.

Bone Marrow Depression/Low Blood Counts

Major:

Thymoglobulin

The anti-thymocyte globulin (rabbit) in Thymoglobulin has a major potential interaction with Bone Marrow Depression/Low Blood Counts.

The use of anti-thymocyte globulin [rabbit] is contraindicated in patients who have active acute or chronic infections that contraindicate any additional immunosuppression. To prevent over-immunosuppression, physicians may wish to decrease the dose of the maintenance immunosuppression regimen during the period of therapy use. Low counts of platelets and white blood cells (including low counts of lymphocytes and neutrophils) have been identified and are reversible following dose adjustments. It is recommended to monitor patients carefully and administer appropriate anti-infective treatment when indicated. Also, total white blood cell and platelet counts should be monitored.

Depression

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Depression.

Failure to Thrive

Moderate:

roflumilast

Roflumilast has a moderate potential interaction with Failure to Thrive.

Weight loss is a common adverse reaction with the use of roflumilast. Patients treated with roflumilast should have their weight monitored regularly. If unexplained or clinically significant weight loss occurs, weight loss should be evaluated, and discontinuation of treatment should be considered. Care should be exercised when using this agent in patients at risk of weight loss.

Immunodeficiency

Major:

Thymoglobulin

The anti-thymocyte globulin (rabbit) in Thymoglobulin has a major potential interaction with Immunodeficiency.

Infection

Major:

Thymoglobulin

The anti-thymocyte globulin (rabbit) in Thymoglobulin has a major potential interaction with Infection.

Liver Disease

Major:

roflumilast

Roflumilast has a major potential interaction with Liver Disease.

Roflumilast is contraindicated in patients with moderate to severe liver impairment (Child-Pugh B or C). Roflumilast is extensively metabolized via Phase I (cytochrome P450) and Phase II (conjugation) reactions. Clinicians should consider the risk-benefit of administering roflumilast to patients with mild liver impairment (Child-Pugh A).

Vaccination

Major:

Thymoglobulin

The anti-thymocyte globulin (rabbit) in Thymoglobulin has a major potential interaction with Vaccination.

Patients recently treated with anti-thymocyte globulin [rabbit] should not receive immunizations with live viral vaccines.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
