MONITOR: Theoretically, the complement inhibitors eculizumab, ravulizumab, and zilucopan may reduce the complement-dependent pharmacodynamic effects of rituximab. The proposed mechanism is related to the potential inhibitory effect of complement inhibitors on complement-dependent cytotoxicity of rituximab. However, clinical data evaluating the interaction are not available and the significance of this effect is unknown.
MANAGEMENT: Until more information is available, caution and closer monitoring for decreased efficacy of rituximab is advised if the complement inhibitors eculizumab, ravulizumab, or zilucopan are used concurrently.
