Drug interactions

Major drug interactions

Major:

delavirdine pemigatinib

Applies to: Rescriptor (delavirdine) and pemigatinib

Delavirdine may increase the blood levels of pemigatinib. This may increase the frequency and severity of serious side effects such as elevated phosphate levels in the blood (which can eventually lead to low blood calcium levels; calcium deposits in the skin, muscles, and other tissues; anemia; muscle cramps; seizures; and irregular heart rhythm), eye and vision problems, joint pain, mouth sores and inflammation, hair loss, diarrhea, nausea, vomiting, and constipation. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Food/lifestyle interactions

Major food/lifestyle interactions

Major:

pemigatinib Food/Lifestyle

Applies to: pemigatinib

Pemigatinib may be taken with or without food. Do not consume grapefruit, grapefruit juice, or any supplements that contain grapefruit extract during treatment with pemigatinib unless directed otherwise by your doctor. Grapefruit juice can increase the blood levels of pemigatinib. Do not stop using any medications without first talking to your doctor.

Disease interactions

5 potential interactions Applies to: 4 conditions, including Liver Disease, Visual Defect/Disturbance, Renal Dysfunction, and Achlorhydria.

Achlorhydria

Moderate:

Rescriptor

The delavirdine in Rescriptor has a moderate potential interaction with Achlorhydria.

Patients with achlorhydria should take delavirdine with an acidic beverage such as orange or cranberry juice, presumably to improve absorption. Although the effects of acidic beverages on delavirdine absorption have not been studied, delavirdine is known to be poorly soluble when pH is greater than 3.

Liver Disease

Moderate:

pemigatinib

Pemigatinib has a moderate potential interaction with Liver Disease.

The recommended dosage of pemigatinib should be reduced in patients with severe hepatic dysfunction. No dose adjustment is recommended for patients with mild or moderate hepatic impairment.

Moderate:

Rescriptor

The delavirdine in Rescriptor has a moderate potential interaction with Liver Disease.

Delavirdine is primarily metabolized by the liver. Patients with liver disease may be at greater risk for toxicities and adverse effects from delavirdine due to decreased drug clearance. Therapy with delavirdine should be administered cautiously in patients with liver disease.

Renal Dysfunction

Moderate:

pemigatinib

Pemigatinib has a moderate potential interaction with Renal Dysfunction.

The recommended dosage of pemigatinib should be reduced in patients with severe renal disease. No dose adjustment is recommended for patients with mild or moderate renal impairment, or those patients with end-stage renal disease receiving intermittent hemodialysis.

Visual Defect/Disturbance

Moderate:

pemigatinib

Pemigatinib has a moderate potential interaction with Visual Defect/Disturbance.

Pemigatinib can cause retinal pigment epithelial detachment (RPED), which may cause symptoms such as blurred vision, visual floaters, or photopsia. A comprehensive ophthalmological examination including an optical coherence tomography should be performed prior to treatment initiation. Patients with visual disturbances should be closely monitored.

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
