Drug interactions

Major drug interactions

Major:

mipomersen tepotinib

Applies to: mipomersen and tepotinib

Mipomersen may cause liver problems, and using it with other medications that can also affect the liver such as tepotinib may increase that risk. You should avoid or limit the use of alcohol to no more than one drink per day while being treated with these medications. Call your doctor immediately if you have fever, chills, joint pain or swelling, unusual bleeding or bruising, skin rash, itching, loss of appetite, fatigue, nausea, vomiting, abdominal pain, dark urine, pale stools, and/or yellowing of the skin or eyes, as these may be signs and symptoms of liver damage. Talk to your doctor or pharmacist if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Moderate food/lifestyle interactions

Moderate:

tepotinib Food/Lifestyle

Applies to: tepotinib

Food helps with the absorption of tepotinib into the blood stream. You should take this medication with food at approximately the same time each day.

Disease interactions

5 potential interactions Applies to: 3 conditions, including Liver Disease, Renal Dysfunction, and Pulmonary Impairment.

Liver Disease

Major:

mipomersen

Mipomersen has a major potential interaction with Liver Disease.

Mipomersen is contraindicated in patients with moderate or severe hepatic impairment (Child-Pugh B or C) or active liver disease, including unexplained persistent elevations of serum transaminases. This agent can cause elevations in transaminases and hepatic steatosis, and could induce steatohepatitis, which could progress to cirrhosis. It is recommended to measure a full liver panel to include ALT, AST, total bilirubin, and alkaline phosphatase before initiation of treatment. If transaminase elevations are accompanied by clinical symptoms of liver injury (e.g., nausea, vomiting, abdominal pain, fever, jaundice, lethargy, flu-like symptoms), increases in bilirubin >= 2 times ULN, or active liver disease, discontinue treatment and identify the probable cause.

Moderate:

tepotinib

Tepotinib has a moderate potential interaction with Liver Disease.

The use of tepotinib may cause hepatotoxicity, sometimes resulting in fatalities. No dosage modification is recommended in patients with mild (Child-Pugh A) or moderate (Child-Pugh B) liver dysfunction. Care should be exercised when using tepotinib in patients with severe liver dysfunction (Child-Pugh C) as its pharmacokinetics and safety have not been studied in this patient population. It is recommended to monitor liver function tests (including ALT, AST, and total bilirubin) prior to the start of therapy, every 2 weeks during the first 3 months of therapy, then once a month or as clinically indicated, with more frequent testing in patients who develop increased transaminases or bilirubin. Tepotinib should be withheld, the dose should be reduced, or therapy should be permanently discontinued based on the severity of the adverse reaction.

Pulmonary Impairment

Moderate:

tepotinib

Tepotinib has a moderate potential interaction with Pulmonary Impairment.

The use of tepotinib may cause interstitial lung disease (ILD)/pneumonitis, sometimes resulting in fatalities. Care should be exercised when using this drug in patients with preexisting pulmonary impairment. Tepotinib should be immediately withheld in patients with suspected ILD/pneumonitis and permanently discontinued if no other potential causes of ILD/pneumonitis are identified. Patients should be monitored for new/worsening pulmonary symptoms indicative of ILD/pneumonitis.

Renal Dysfunction

Moderate:

mipomersen

Mipomersen has a moderate potential interaction with Renal Dysfunction.

The use of mipomersen is not recommended in patients with severe renal impairment, clinically significant proteinuria or on renal dialysis. The safety and efficacy of this agent has not been established in patients with renal impairment or undergoing renal dialysis. Care should be exercised when using this agent in patients with renal dysfunction.

Moderate:

tepotinib

Tepotinib has a moderate potential interaction with Renal Dysfunction.

Care should be exercised when using tepotinib in patients with severe renal dysfunction (CrCl less than 30 mL/min) as the recommended dosage has not been established in this patient population. No dosage adjustment is recommended in patients with mild or moderate renal dysfunction (CrCl 30 to 89 mL/min).

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
