MONITOR: Concomitant use of sparsentan and antihypertensive drugs or drugs with hypotensive properties may result in additive blood pressure-lowering effects. Sparsentan is an endothelin and angiotensin II receptor antagonist. Hypotension has been observed in patients treated with angiotensin II receptor antagonists (ARBs) and endothelin receptor antagonists (ERAs) and was observed in clinical studies with sparsentan. In PROTECT, the randomized, double-blind, active-controlled clinical study in adults with primary immunoglobulin A nephropathy (IgAN), there was a greater incidence of significant hypotension-related adverse events, including orthostatic hypotension and dizziness in those treated with sparsentan compared to irbesartan.
MANAGEMENT: Caution and monitoring for hypotension and appropriate volume status is recommended if sparsentan is used in combination with antihypertensive medication or other medication with hypotensive effects. A dosage reduction or discontinuation of concomitant antihypertensive therapy should also be considered in patients at risk for hypotension. If hypotension develops despite dose reduction or cessation of concomitant antihypertensive therapy, a dose reduction or dose interruption of sparsentan should be considered. If the hypotensive response is transient, sparsentan may be administered once blood pressure has stabilized. Patients should be advised to contact their physician if they experience symptoms of hypotension such as dizziness, lightheadedness, or fainting.

If you are taking sparsentan you should avoid potassium-containing salt substitutes or over-the-counter potassium supplements without first talking to your doctor. This can cause high levels of potassium in your blood. High levels of potassium can cause weakness, irregular heartbeat, confusion, tingling of the extremities, or feelings of heaviness in the legs. Call your doctor at once if you have any of these symptoms.
Consumption of grapefruit, grapefruit juice, and supplements that contain grapefruit should be avoided during treatment with sparsentan as they may increase the blood levels of sparsentan. This may increase the risk of side effects such as hepatotoxicity, acute kidney injury, hyperkalemia, edema, and hypotension.
Swallow sparsentan whole with water before your morning or evening meal. Take your dose with the same meal each day. Talk to your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
