ADJUST DOSE: Coadministration with dronedarone may significantly increase the serum concentrations of digoxin. The proposed mechanism is dronedarone inhibition of the P-glycoprotein-mediated renal tubular secretion of digoxin. According to the product labeling, dronedarone increased digoxin systemic exposure (AUC) by 2.5-fold. Gastrointestinal disorders were increased during coadministration in clinical trials for dronedarone. Conversely, digoxin can potentiate the electrophysiologic effects of dronedarone, such as decreased AV node conduction. In separate studies of patients with recently decompensated heart failure and patients with permanent atrial fibrillation, baseline use of digoxin was associated with an increased risk of arrhythmic or sudden death in patients receiving dronedarone compared to those receiving placebo. In patients not taking digoxin, no difference in risk of sudden death was observed in the dronedarone group relative to the placebo group.

MANAGEMENT: The need for digoxin therapy should be reconsidered prior to initiating dronedarone. If coadministration is deemed necessary, the digoxin dose should be reduced by one-half, and serum digoxin levels and pharmacologic effects closely monitored. Patients should be advised to notify their physician if they experience signs of digoxin toxicity such as nausea, anorexia, visual disturbances, slow pulse, or irregular heartbeats.

GENERALLY AVOID: Grapefruit juice may increase the plasma concentrations of dronedarone. The proposed mechanism is inhibition of CYP450 3A4-mediated first-pass metabolism in the gut wall by certain compounds present in grapefruits. According to the product labeling, administration with grapefruit juice resulted in a 2.5-fold increase in dronedarone peak plasma concentration and a 3-fold increase in systemic exposure. Because dronedarone is associated with concentration-dependent prolongation of the QT interval, increased levels may potentiate the risk of ventricular arrhythmias such as torsade de pointes and sudden death.

ADJUST DOSING INTERVAL: Food increases the oral bioavailability of dronedarone. The mechanism of interaction is unknown. According to the product labeling, the absolute bioavailability of dronedarone increases from about 4% when administered in the fasted state to approximately 15% when administered with a high-fat meal.

MANAGEMENT: Patients treated with dronedarone should avoid consumption of grapefruit, grapefruit juice, and any supplement containing grapefruit extract. Dronedarone should be taken twice daily with the morning and evening meals.

digoxin food

Administration of digoxin with a high-fiber meal has been shown to decrease its bioavailability by almost 20%. Fiber can sequester up to 45% of the drug when given orally. Patients should be advised to maintain a regular diet without significant fluctuation in fiber intake while digoxin is being titrated.

Grapefruit juice may modestly increase the plasma concentrations of digoxin. The mechanism is increased absorption of digoxin due to mild inhibition of intestinal P-glycoprotein by certain compounds present in grapefruits. In 12 healthy volunteers, administration of grapefruit juice with and 30 minutes before, as well as 3.5, 7.5, and 11.5 hours after a single digoxin dose (0.5 mg) increased the mean area under the plasma concentration-time curve (AUC) of digoxin by just 9% compared to administration with water. Moreover, P-glycoprotein genetic polymorphism does not appear to influence the magnitude of the effects of grapefruit juice on digoxin. Thus, the interaction is unlikely to be of clinical significance.
