Drug Interactions between Denzapine and Diamox

acetaZOLAMIDE cloZAPine
Applies to: Diamox (acetazolamide) and Denzapine (clozapine)
MONITOR: One case report has suggested that combination of carbonic anhydrase inhibitors (CAIs) and clozapine may increase the risk of neutropenia. The case involved methazolamide, and the authors speculate that acetazolamide may participate in this interaction as well.
MANAGEMENT: Close monitoring is advisable during concomitant use. White blood cell (WBC) counts and absolute neutrophil counts (ANC) should be measured regularly according to official recommendations, and clozapine should be discontinued if cell counts fall below critical limits. Patients should be counseled to notify their doctor immediately if they experience influenza-like symptoms, fever, or a sore throat.

cloZAPine food
Applies to: Denzapine (clozapine)
GENERALLY AVOID: Alcohol may potentiate some of the pharmacologic effects of CNS-active agents. Use in combination may result in additive central nervous system depression and/or impairment of judgment, thinking, and psychomotor skills.
MANAGEMENT: Patients receiving CNS-active agents should be warned of this interaction and advised to avoid or limit consumption of alcohol. Ambulatory patients should be counseled to avoid hazardous activities requiring complete mental alertness and motor coordination until they know how these agents affect them, and to notify their physician if they experience excessive or prolonged CNS effects that interfere with their normal activities.

cloZAPine food
Applies to: Denzapine (clozapine)
Caffeine may increase clozapine serum concentrations and exacerbate psychotic symptoms. The mechanism is unknown but may be related to competition for the same metabolic pathway. No specific intervention is necessary; however, if an interaction is suspected it is recommended that caffeine intake be avoided.
