Interactions between Deferiprone and PureFe OB Plus
1 drug interaction and 4 additional warnings found for the following 2 drugs:
- deferiprone
- PureFe OB Plus (multivitamin, prenatal)
Drug interactions
Moderate drug interactions
Moderate:
multivitamins, prenatal deferiprone
Applies to: PureFe OB Plus (multivitamin, prenatal) and deferiprone
Adjust dosing interval
Deferiprone has the potential to bind polyvalent cations such as aluminum, iron and zinc, which may reduce its effectiveness. However, concurrent use with foods, mineral supplements, or antacids that contain polyvalent cations has not been studied.
Management
A minimum 4-hour interval is recommended between the administration of deferiprone and mineral supplements or oral medications that contain antacids (e.g., didanosine buffered tablets or pediatric oral solution) or other polyvalent cations.

Food/lifestyle interactions
Moderate food/lifestyle interactions
Moderate:
multivitamins, prenatal Food/Lifestyle
Applies to: PureFe OB Plus (multivitamin, prenatal)
Adjust dosing interval
Concomitant use of some oral medications may reduce the bioavailability of orally administered iron, and vice versa. Food taken in conjunction with oral iron supplements may reduce the bioavailability of the iron. However, in many patients intolerable gastrointestinal side effects occur necessitating administration with food.
Management
Ideally, iron products should be taken on an empty stomach (i.e., at least 1 hour before or 2 hours after meals), but if this is not possible, administer with meals and monitor the patient more closely for a subtherapeutic effect. Some studies suggest administration of iron with ascorbic acid may enhance bioavailability. In addition, administration of oral iron products and some oral medications should be separated whenever the bioavailability of either agent may be decreased. Consult the product labeling for specific separation times and monitor clinical responses as appropriate.

Disease interactions
3 potential interactions Applies to: 3 conditions, including Liver Disease, Infection, and Neutropenia.
Infection
Major:
deferiprone
Deferiprone has a major potential interaction with Infection.
Fatal agranulocytosis and neutropenia can occur with the use of deferiprone. It is recommended to obtain an absolute neutrophil count before initiating treatment with deferiprone and monitor weekly thereafter or as clinically appropriate. Manage agranulocytosis/neutropenia prior to initiating treatment. Therapy should be interrupted if neutropenia develops or if an infection develops. Do not resume therapy in patients who have developed agranulocytosis and do not rechallenge patients who develop neutropenia unless potential benefits outweigh potential risks.
Liver Disease
Moderate:
deferiprone
Deferiprone has a moderate potential interaction with Liver Disease.
A clinical study to evaluate the effect of impaired hepatic function on the pharmacokinetics of deferiprone showed that mild and moderate hepatic impairment do not influence the pharmacokinetics of deferiprone and major metabolite, deferiprone 3-O-glucuronide. Based on the results, no dose adjustment is required in patients with mildly or moderately hepatic impairment. Caution is recommended when using this agent in patients with severe hepatic impairment as the pharmacokinetics of deferiprone and deferiprone have not been evaluated. It is recommended to monitor serum transaminase values monthly during therapy and to consider interruption if there is a persistent increase in serum transaminase levels. Close monitoring is recommended.
Neutropenia
Major:
deferiprone
Deferiprone has a major potential interaction with Neutropenia.

Drug Interaction Classification
| Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit. | |
| Moderately clinically significant. Usually avoid combinations; use it only under special circumstances. | |
| Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan. | |
| No interaction information available. |
