Narcotics like codeine can commonly cause constipation, which may increase the risk of intestinal problems associated with the use of sodium polystyrene sulfonate, including bleeding, ischemic colitis (inflammation of the intestine due to restricted blood flow), perforation, and necrosis (death of cells and tissues in the intestine). Additional risk factors include prematurity (infants), history of intestinal disease or surgery, dehydration, and impaired kidney function. Before using sodium polystyrene sulfonate, you should let your doctor know if you have chronic constipation, inflammatory bowel disease, previous bowel resection surgery, or other intestinal issues. Stop using sodium polystyrene sulfonate and contact your doctor if you develop constipation during treatment. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

GENERALLY AVOID: Alcohol may potentiate the central nervous system (CNS) depressant effects of opioid analgesics. Concomitant use may result in additive CNS depression and impairment of judgment, thinking, and psychomotor skills. In more severe cases, hypotension, respiratory depression, profound sedation, coma, or even death may occur. In addition, alcohol may affect opioid release from sustained-release formulations.
GENERALLY AVOID: Grapefruit or grapefruit juice may increase the plasma concentrations of opioid analgesics by inhibiting CYP450 3A4-mediated metabolism of these agents, although the interaction has not been studied. In general, the effect of grapefruit juice is concentration-, dose- and preparation-dependent, and can vary widely among brands. Certain preparations of grapefruit juice (e.g., high dose, double strength) have sometimes demonstrated potent inhibition of CYP450 3A4, while other preparations (e.g., low dose, single strength) have typically demonstrated moderate inhibition. Pharmacokinetic interactions involving grapefruit juice are also subject to a high degree of interpatient variability, thus the extent to which a given patient may be affected is difficult to predict.
MANAGEMENT: Patients should not consume alcoholic beverages or use drug products that contain alcohol during treatment with opioid analgesics. Any history of alcohol or illicit drug use should be considered when prescribing an opioid analgesic, and therapy initiated at a lower dosage if necessary. Patients should be closely monitored for signs and symptoms of sedation, respiratory depression, and hypotension. Due to a high degree of interpatient variability with respect to grapefruit juice interactions, patients treated with opioid analgesics should preferably avoid the consumption of grapefruit and grapefruit juice.

Foods high in potassium such as orange juice and other fruit juices may reduce the effectiveness of sodium polystyrene sulfonate in treating your condition. Do not mix sodium polystyrene sulfonate in fruit juice or drink fruit juice with it. Also, sodium polystyrene sulfonate may interfere with the absorption of other medications that you take by mouth, including both prescription medications such as antibiotics, blood pressure/heart medications and blood thinners, and over-the-counter products such as antacids and laxatives. This can lead to lower blood levels and reduced effects if you take them too closely together with sodium polystyrene sulfonate. It is recommended that you separate the dosing of sodium polystyrene sulfonate from other oral medications by at least 3 hours whenever possible. This interval should be increased to 6 hours if you have gastroparesis or other conditions that cause delayed emptying of food from the stomach into the intestine. Sodium polystyrene sulfonate can bind to calcium, iron, magnesium, and other minerals in the gastrointestinal tract. This may inactivate both sodium polystyrene sulfonate and the mineral it binds to and reduce the effectiveness of both medications. To avoid or minimize the interaction, the dosing times of sodium polystyrene sulfonate and multivitamin with minerals should be separated by several hours. Talk to your doctor if you have any questions or concerns, or if you have trouble separating the dosing times. Your doctor may be able to prescribe alternatives that do not interact.
