CONTRAINDICATED: Coadministration of ritonavir-boosted nirmatrelvir may lead to increased plasma concentrations of clozapine. The mechanism may be due to inhibition of clozapine metabolism by nirmatrelvir via CYP450 3A4 and ritonavir via CYP450 3A4 and 2D6. However, no pharmacokinetic data are currently available. Increased plasma concentrations of clozapine increases the risk of adverse effects such as agranulocytosis, seizure, tachycardia, QT prolongation, orthostatic hypotension, somnolence, constipation, and fever.

MANAGEMENT: The manufacturer of nirmatrelvir-ritonavir considers concomitant use with clozapine to be contraindicated.

cloZAPine ritonavir

Applies to: clozapine and nirmatrelvir / ritonavir

MONITOR: Coadministration with ritonavir may theoretically increase the serum concentrations of clozapine. The proposed mechanism is ritonavir inhibition of clozapine metabolism via CYP450 3A4 and 2D6. However, no pharmacokinetic data are currently available. Clozapine is also metabolized by CYP450 1A2. Because it is a substrate for several different CYP450 enzymes, the risk of metabolic interactions caused by an effect on an individual isoform is thought to be minimized. Nevertheless, the manufacturer recommends caution during concomitant treatment of clozapine with other drugs that are inhibitors of these enzymes.

MANAGEMENT: Caution is advised if clozapine is prescribed in combination with ritonavir. Pharmacologic response to clozapine should be monitored more closely whenever ritonavir is added to or withdrawn from therapy in patients stabilized on their existing antipsychotic regimen, and the clozapine dosage adjusted as necessary.

GENERALLY AVOID: Alcohol may potentiate some of the pharmacologic effects of CNS-active agents. Use in combination may result in additive central nervous system depression and/or impairment of judgment, thinking, and psychomotor skills.

MANAGEMENT: Patients receiving CNS-active agents should be warned of this interaction and advised to avoid or limit consumption of alcohol. Ambulatory patients should be counseled to avoid hazardous activities requiring complete mental alertness and motor coordination until they know how these agents affect them, and to notify their physician if they experience excessive or prolonged CNS effects that interfere with their normal activities.

ADJUST DOSING INTERVAL: Administration with food may modestly affect the bioavailability of ritonavir from the various available formulations. When the oral solution was given under nonfasting conditions, peak ritonavir concentrations decreased 23% and the extent of absorption decreased 7% relative to fasting conditions. Dilution of the oral solution (within one hour of dosing) with 240 mL of chocolate milk or a nutritional supplement (Advera or Ensure) did not significantly affect the extent and rate of ritonavir absorption. When a single 100 mg dose of the tablet was administered with a high-fat meal (907 kcal; 52% fat, 15% protein, 33% carbohydrates), approximately 20% decreases in mean peak concentration (Cmax) and systemic exposure (AUC) were observed relative to administration after fasting. Similar decreases in Cmax and AUC were reported when the tablet was administered with a moderate-fat meal. In contrast, the extent of absorption of ritonavir from the soft gelatin capsule formulation was 13% higher when administered with a meal (615 KCal; 14.5% fat, 9% protein, and 76% carbohydrate) relative to fasting.

MANAGEMENT: Ritonavir should be taken with meals to enhance gastrointestinal tolerability.

MONITOR: Smoking cessation may lead to elevated plasma concentrations and enhanced pharmacologic effects of drugs that are substrates of CYP450 1A2 (and possibly CYP450 1A1) and/or certain drugs with a narrow therapeutic index (e.g., flecainide, pentazocine). One proposed mechanism is related to the loss of CYP450 1A2 and 1A1 induction by polycyclic aromatic hydrocarbons in tobacco smoke; when smoking cessation agents are initiated and smoking stops, the metabolism of certain drugs may decrease leading to increased plasma concentrations. The mechanism by which smoking cessation affects narrow therapeutic index drugs that are not known substrates of CYP450 1A2 or 1A1 is unknown. The clinical significance of this interaction is unknown as clinical data are lacking.

MANAGEMENT: Until more information is available, caution is advisable if smoking cessation agents are used concomitantly with drugs that are substrates of CYP450 1A2 or 1A1 and/or those with a narrow therapeutic range. Patients receiving smoking cessation agents may require periodic dose adjustments and closer clinical and laboratory monitoring of medications that are substrates of CYP450 1A2 or 1A1.

Caffeine may increase clozapine serum concentrations and exacerbate psychotic symptoms. The mechanism is unknown but may be related to competition for the same metabolic pathway. No specific intervention is necessary; however, if an interaction is suspected it is recommended that caffeine intake be avoided.
