MONITOR: Coadministration of clobazam with inhibitors of CYP450 2C19 may significantly increase the plasma concentrations of its active metabolite, N-desmethylclobazam, which is primarily metabolized by the isoenzyme. Based on extrapolation from pharmacogenomic data, inhibition of CYP450 2C19 may result in up to a 5-fold increase in exposure to N-desmethylclobazam. At therapeutic dosages of clobazam, plasma concentrations of N-desmethylclobazam are 3 to 5 times higher than those of the parent compound. Based on animal and in vitro receptor binding data, estimates of the relative potency of N-desmethylclobazam compared to parent compound range from 1/5 to equal potency.
MANAGEMENT: Dosage adjustment of clobazam may be necessary when coadministered with strong (e.g., fluconazole, fluvoxamine, ticlopidine) or moderate (e.g., omeprazole) inhibitors of CYP450 2C19.

You should avoid or limit the use of alcohol while being treated with cloBAZam. Do not use more than the recommended dose of cloBAZam, and avoid activities requiring mental alertness such as driving or operating hazardous machinery until you know how the medication affects you.
