ADJUST DOSE: The coadministration with grepafloxacin may increase the serum concentrations of theophylline. The mechanism is grepafloxacin inhibition of CYP450 1A2, the isoenzyme primarily responsible for the metabolic clearance of theophylline. In 12 healthy volunteers maintained on theophylline to produce a mean serum concentration of 7.5 mg/L, administration of grepafloxacin (600 mg orally once a day for 10 days) increased the mean peak serum concentration (Cmax) and area under the concentration-time curve (AUC) of theophylline by about 80% and 100%, respectively. Apparent total clearance (Cl/F) was reduced by approximately 50%. Theophylline had no effect on the pharmacokinetics of grepafloxacin.

MANAGEMENT: A 50% reduction in theophylline maintenance dosage is recommended during grepafloxacin treatment, and serum theophylline levels should be monitored to further guide any therapy adjustments. Patients should be advised to notify their physician if they experience possible signs of theophylline toxicity such as nausea, vomiting, diarrhea, headache, restlessness, insomnia, seizures, or irregular heartbeats.
