MONITOR: Coadministration with cenobamate may decrease the plasma concentrations and pharmacologic effects of CYP450 2B6 substrates. The proposed mechanism is cenobamate-mediated induction of the CYP450 2B6 isoenzyme. According to the manufacturer, multiple doses of cenobamate (200 mg once daily) reduced the mean Cmax and AUC of bupropion, a CYP450 2B6 substrate, by 23% and 39%, respectively.

MANAGEMENT: Caution is advised if cenobamate is used concomitantly with drugs that are substrates of CYP450 2B6, particularly those with a narrow therapeutic index. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever cenobamate is added to or withdrawn from therapy.

One study (n=12) has reported that there was a significant interactive effect of caffeine and nicotine on subjective arousal such that nicotine decreased arousal only in the presence of caffeine. The exact mechanism of this interaction has not been specified. Further research is needed to examine the subjective and physiological interactions between caffeine and nicotine.
