Drug interactions
Moderate drug interactions
Moderate:
calaspargase pegol eflornithine
Applies to: calaspargase pegol and eflornithine
Monitor
Concomitant use of asparaginase with other hepatotoxic agents may potentiate the risk of liver injury. Asparaginase-associated hepatotoxicity has been reported more commonly in adults than in children and has been strongly associated with obesity. Hepatomegaly, acute severe hepatotoxicity, and fatal liver failure have been reported with asparaginase treatment in adults. Also, asparaginase may increase the toxicity of drugs bound to plasma proteins or metabolized by the liver.
Management
The risk of additive hepatotoxicity should be considered when asparaginase is used with other hepatotoxic agents (e.g., alcohol, androgens, antituberculosis agents, azole antifungal agents, ACE inhibitors, macrolide antibiotics, nonsteroidal anti-inflammatory agents, nucleoside reverse transcriptase inhibitors, sulfonamides, thiazolidinediones, and statins). Liver function tests should be monitored at regular intervals during asparaginase treatment with or without other hepatotoxic drugs. Patients should be advised to seek medical attention if they experience potential symptoms of hepatotoxicity such as right upper quadrant pain, increasing abdominal size, fever, rash, itching, anorexia, nausea, vomiting, fatigue, malaise, dark urine, pale stools, and jaundice.

Food/lifestyle interactions
No alcohol/food interactions were found. However, this does not necessarily mean no interactions exist. Always consult your healthcare provider.
Disease interactions
7 potential interactions Applies to: 6 conditions, including Hearing Loss, Liver Disease, Pancreatitis, Bleeding Disorder, and Renal Dysfunction.
Bleeding Disorder
Major:
calaspargase pegol
Calaspargase pegol has a major potential interaction with Bleeding Disorder.
Hemorrhage associated with increased prothrombin time (PT), increased partial thromboplastin time (PTT), and hypofibrinogenemia have been reported in patients receiving calaspargase pegol. Evaluate patients with signs and symptoms of hemorrhage with coagulation parameters including PT, PTT, fibrinogen. Consider appropriate replacement therapy in any patients with severe or symptomatic coagulopathy. The use of calaspargase pegol is contraindicated in patients with any history of serious hemorrhagic events during a previous L-asparaginase therapy.
Coagulation Defects and Disorders
Major:
calaspargase pegol
Calaspargase pegol has a major potential interaction with Coagulation Defects and Disorders.
Hearing Loss
Moderate:
eflornithine
Eflornithine has a moderate potential interaction with Hearing Loss.
Eflornithine can cause hearing loss. New or worsening hearing loss requiring new use of hearing aids occurred in 7% of patients in clinical trials. An audiogram should be performed prior to initiation of therapy and at 6 month intervals, or as clinically indicated, to monitor new or worsening hearing loss.
Liver Disease
Major:
calaspargase pegol
Calaspargase pegol has a major potential interaction with Liver Disease.
The use of calaspargase pegol is contraindicated in patients with severe hepatic impairment. Caution and monitoring is advised in patients with liver impairment as hepatotoxicity and abnormal liver function, including elevations of transaminase, bilirubin (direct and indirect), reduced serum albumin, and plasma fibrinogen can occur. Evaluate bilirubin and transaminases at least weekly, during cycles of treatment that include calaspargase pegol, through 6 weeks after the last dose. In the event of serious liver toxicity, discontinue treatment and provide supportive care.
Moderate:
eflornithine
Eflornithine has a moderate potential interaction with Liver Disease.
The pharmacokinetics of eflornithine have not been studied in patients with moderate or severe liver dysfunction.
Pancreatitis
Major:
calaspargase pegol
Calaspargase pegol has a major potential interaction with Pancreatitis.
Cases of pancreatitis have been reported in clinical trials with calaspargase pegol with an incidence as high as 16%. Caution is advised if used in patients with a history of pancreatitis. Inform patients of the signs and symptoms of pancreatitis, which, if left untreated, could be fatal. Assess serum amylase and/or lipase levels to identify early signs of pancreatic inflammation. Discontinue calaspargase pegol if pancreatitis is suspected; if pancreatitis is confirmed, do not resume treatment. Calaspargase pegol is contraindicated in patients with a history of pancreatitis due to any previous asparaginase therapy.
Renal Dysfunction
Moderate:
eflornithine
Eflornithine has a moderate potential interaction with Renal Dysfunction.
Patients with moderate (eGFR <60 mL/min) and severe (eGFR <30 mL/min) renal impairment have a higher exposure to eflornithine than patients with normal renal function which can increase the risk of adverse effects and toxicity. Reduce the dose by 50% in patients with severe renal impairment. Monitor patients closely for increased adverse reactions including hepatotoxicity, myelosuppression, and hearing loss.
