Using buprenorphine together with other medications that also cause central nervous system depression can lead to serious side effects such as respiratory distress, coma, and even death. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. Do not drink alcohol or self-medicate with these medications without your doctor's approval, and do not exceed the doses or frequency and duration of use prescribed by your doctor. Using samidorphan together with buprenorphine is not recommended. Samidorphan can block the effects of opioids like buprenorphine and make the medication less effective or ineffective in treating your condition. If you have been receiving opioid-containing medications for a while (for example, a week or longer), initiating samidorphan can also precipitate withdrawal symptoms that sometimes are severe enough to warrant hospitalization. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or direct you to stop using buprenorphine for at least one to two weeks before you initiate treatment with samidorphan. Do not increase the dose or frequency of use of buprenorphine on your own in order to overcome the opioid-blocking effects of samidorphan, as this may increase the risk of opioid overdose and death, especially when samidorphan is interrupted or discontinued. Following treatment with samidorphan, it takes some time for the opioid-blocking effect to go away and you may be more more sensitive to the effects of opioids during this time. If you have used opioid street drugs or opioid-containing medications in the past, using them in the amounts that you used before treatment with samidorphan can also lead to overdose and death. Therefore, you should wait at least 5 days after you stop treatment with samidorphan before you start using opioids. However, it is best to consult a healthcare provider before using any opioids. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
