- Bleo 15k (bleomycin)
- omacetaxine
Drug interactions
Moderate drug interactions
Moderate:
bleomycin omacetaxine
Applies to: Bleo 15k (bleomycin) and omacetaxine
Using omacetaxine together with bleomycin may increase the risk of serious infections. Talk to your doctor if you have any questions or concerns. Your doctor may already be aware of the risks, but has determined that this is the best course of treatment for you and has taken appropriate precautions and is monitoring you closely for any potential complications. Contact your doctor immediately if you develop signs and symptoms of infection such as fever, chills, diarrhea, sore throat, muscle aches, shortness of breath, blood in phlegm, weight loss, red or inflamed skin, body sores, and pain or burning during urination. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
Food/lifestyle interactions
No alcohol/food interactions were found. However, this does not necessarily mean no interactions exist. Always consult your healthcare provider.
Disease interactions
7 potential interactions Applies to: 7 conditions, including Diabetes Mellitus, Liver Disease, Bleeding Disorder, Thrombocytopenia, and Renal Dysfunction.
Bleeding Disorder
Moderate:
omacetaxine
Omacetaxine has a moderate potential interaction with Bleeding Disorder.
Omacetaxine can cause severe thrombocytopenia. Bleeding events, including fatalities from cerebral hemorrhage, and severe, non-fatal, gastrointestinal hemorrhages have been reported in clinical trials. Perform complete blood counts, including platelet counts weekly during induction and initial maintenance cycles. It is recommended to avoid anticoagulants, aspirin, and nonsteroidal anti-inflammatory drugs (NSAIDs) when the platelet count is less than 50,000/µL as they may increase the risk of bleeding. Caution is recommended when using this agent in patients at risk of bleeding episodes.
Bone Marrow Depression/Low Blood Counts
Moderate:
omacetaxine
Omacetaxine has a moderate potential interaction with Bone Marrow Depression/Low Blood Counts.
The use of omacetaxine might cause hematological toxicities. Patients with neutropenia are at increased risk for infections, and should be monitored frequently and advised to contact a physician if they have symptoms of infection or fever. It is recommended to monitor complete blood counts (CBCs) weekly during induction and initial maintenance cycles and every two weeks during later maintenance cycles, as clinically indicated. After initial maintenance cycles, monitor CBCs every two weeks or as clinically indicated. Care should be exercised when using omacetaxine in patients with hematological disorders.
Diabetes Mellitus
Moderate:
omacetaxine
Omacetaxine has a moderate potential interaction with Diabetes Mellitus.
The use of omacetaxine can induce glucose intolerance. Hyperglycemia has been reported in patients using omacetaxine. It is recommended to monitor blood glucose levels frequently, especially in patients with a history of diabetes or in those with risk factors for diabetes. Avoid the use of omacetaxine in patients with poorly controlled diabetes.
Liver Disease
Moderate:
Bleo 15k
The bleomycin in Bleo 15k has a moderate potential interaction with Liver Disease.
The effect of hepatic insufficiency on the pharmacokinetics of bleomycin has not been evaluated. However, hepatic toxicity, beginning as a deterioration in liver function tests, has been reported. These toxicities may occur at any time after initiation of therapy. Caution and monitoring is advised if bleomycin is used in patients with liver impairment.
Pulmonary Impairment
Major:
Bleo 15k
The bleomycin in Bleo 15k has a major potential interaction with Pulmonary Impairment.
Pulmonary pneumonitis occurs in 10% of patients during bleomycin therapy and 1% progress to pulmonary fibrosis and death. Although pulmonary toxicity is age and dose related, occurring primarily in patients over 70 years of age and in those receiving a total dose > 400 units, it has occurred in younger patients at a lower dose. Extreme caution should be exercised when administering bleomycin in patients with compromised pulmonary function. Patients should be instructed to immediately report symptoms of dyspnea, cough, or congestion. Close clinical monitoring of pulmonary function is recommended.
Renal Dysfunction
Major:
Bleo 15k
The bleomycin in Bleo 15k has a major potential interaction with Renal Dysfunction.
Bleomycin is primarily eliminated by the kidney. Approximately 60% to 70% of bleomycin is excreted in the urine as active drug. Moderate renal failure reduces elimination to < 20% of the drug. Extreme caution should be exercised when administering bleomycin in patients with existing or predisposition to significantly compromised renal function. Close clinical monitoring of renal function is recommended.
Thrombocytopenia
Moderate:
omacetaxine
Omacetaxine has a moderate potential interaction with Thrombocytopenia.

| Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit. | |
| Moderately clinically significant. Usually avoid combinations; use it only under special circumstances. | |
| Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan. | |
| No interaction information available. |
