MONITOR: Acetylcholinesterase inhibitors may have vagotonic effects on the sinoatrial and atrioventricular nodes, which occasionally manifest as bradycardia or heart block (<2%). Because bradycardia is a risk factor for torsade de pointes, a theoretical risk exists when combined with agents that prolong the QT interval. In general, the risk of an individual agent or a combination of agents causing ventricular arrhythmia in association with QT prolongation is largely unpredictable but may be increased by certain underlying risk factors including, but not limited to congenital long QT syndrome, cardiac disease, and electrolyte disturbances (e.g., hypokalemia, hypomagnesemia). In addition, the extent of drug-induced QT prolongation may vary depending on the dosage(s) and specific drug(s) involved.

MANAGEMENT: Caution is advised when acetylcholinesterase inhibitors are used with drugs that can prolong the QT interval. Patients should be monitored for bradycardia, atrioventricular block and syncope, and advised to seek medical attention if they experience dizziness, lightheadedness, fainting, shortness of breath, or slow or irregular heartbeat.

MONITOR: Coadministration with lumefantrine may increase the plasma concentrations of drugs that are substrates of the CYP450 2D6 isoenzyme. In vitro, lumefantrine significantly inhibits the activity of CYP450 2D6 at therapeutic plasma concentrations. Adverse effects of drugs that are primarily metabolized by CYP450 2D6 may be increased; however, clinical data have not been reported.

MANAGEMENT: Caution is advised if lumefantrine is used concurrently with medications that undergo metabolism by CYP450 2D6, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever lumefantrine is added to or withdrawn from therapy.

Each dose of lumefantrine should be taken with food such as milk, infant formula, pudding, porridge, or broth. Food helps the medication get absorbed into the bloodstream, so the medication may not work as well when taken on an empty stomach. If unable to be swallowed whole, lumefantrine tablets may be crushed and mixed with one to two teaspoons of water in a clean container and consumed immediately after mixing. The container should then be rinsed with more water and the contents consumed. This should be followed by eating as soon as possible. Avoid drinking grapefruit juice during treatment with lumefantrine. Grapefruit juice can cause too much of the medication to be in the blood. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
