Tocainide causes a decrease in theophylline clearance. The result is an increase in serum theophylline concentrations and a mild increase in the serum half-life of theophylline. The magnitude of this interaction is less than for mexiletine, which has been shown to reduce theophylline plasma clearance by 40% to 45%. The mechanism of this interaction is not known. Tocainide may interact in a similar way with other xanthines. Management consists of monitoring clinical status during therapy and checking serum theophylline levels as necessary. Patients should be advised to report any signs of theophylline toxicity including nausea, vomiting, diarrhea, headache, restlessness, insomnia, or irregular heartbeat to their physician.

MONITOR: Coadministration of two or more sympathomimetic agents may increase the risk of adverse effects such as nervousness, irritability, and increased heart rate. Central nervous system (CNS) stimulants, particularly amphetamines, can potentiate the adrenergic response to vasopressors and other sympathomimetic agents. Additive increases in blood pressure and heart rate may occur due to enhanced peripheral sympathetic activity.

MANAGEMENT: Caution is advised if two or more sympathomimetic agents are coadministered. Pulse and blood pressure should be closely monitored.
