Applies to: aluminum carbonate and Osteo Bi-Flex Joint Health (boswellia / cholecalciferol / glucosamine)

GENERALLY AVOID: Chronic use of aluminum-containing preparations in patients with advanced renal impairment, particularly during treatment with vitamin D analogs, may result in increased blood levels of aluminum and aluminum toxicity. Patients with renal failure are at risk for toxicity due to impaired clearance of aluminum, and the risk may be further increased by the administration of vitamin D analogs due to increased intestinal absorption of aluminum. Toxicity may manifest as osteodystrophy, osteomalacia, myopathy, anemia, and encephalopathy.

MANAGEMENT: Aluminum-containing preparations (e.g., antacids, phosphate binders) should not be administered chronically with vitamin D analogs.

GENERALLY AVOID: The concomitant administration of aluminum-containing products (e.g., antacids and phosphate binders) and citrates may significantly increase serum aluminum concentrations, resulting in toxicity. Citrates or citric acid are contained in numerous soft drinks, citrus fruits, juices, and effervescent and dispersible drug formulations. Citrates enhance the gastrointestinal absorption of aluminum by an unknown mechanism, which may involve the formation of a soluble aluminum-citrate complex. Various studies have reported that citrate increases aluminum absorption by 4.6- to 50-fold in healthy subjects. Patients with renal insufficiency are particularly at risk of developing hyperaluminemia and encephalopathy. Fatalities have been reported. Patients with renal failure or on hemodialysis may also be at risk from soft drinks and effervescent and dispersible drug formulations that contain citrates or citric acid. It is unknown what effect citrus fruits or juices would have on aluminum absorption in healthy patients.

MANAGEMENT: The concomitant use of aluminum- and citrate-containing products and foods should be avoided by renally impaired patients. Hemodialysis patients should especially be cautioned about effervescent and dispersible over-the-counter remedies and soft drinks. Some experts also recommend that healthy patients should separate doses of aluminum-containing antacids and citrates by 2 to 3 hours.

ADJUST DOSING INTERVAL: The administration of aluminum-containing antacids with enteral nutrition may result in precipitation, formation of bezoars, and obstruction of feeding tubes. The proposed mechanism is the formation of an insoluble complex between the aluminum and the protein in the enteral feeding. Several cases of esophageal plugs and nasogastric tube obstructions have been reported in patients receiving high-protein liquids and an aluminum hydroxide-magnesium hydroxide antacid or an aluminum hydroxide antacid.

MANAGEMENT: Some experts recommend that antacids should not be mixed with or given after high protein formulations, that the antacid dose should be separated from the feeding by as much as possible, and that the tube should be thoroughly flushed before administration.

MONITOR: Additive effects and possible toxicity (e.g., hypercalcemia, hypercalciuria, and/or hyperphosphatemia) may occur when patients using vitamin D and/or vitamin D analogs ingest a diet high in vitamin D, calcium, and/or phosphorus. The biologically active forms of vitamin D stimulate intestinal absorption of calcium and phosphorus. This may be helpful in patients with hypocalcemia and/or hypophosphatemia. However, sudden increases in calcium or phosphorus consumption due to dietary changes could precipitate hypercalcemia and/or hyperphosphatemia. Patients with certain disease states, such as impaired renal function, may be more susceptible to toxic side effects like ectopic calcification. On the other hand, if dietary calcium is inadequate for the body's needs, the active form of vitamin D will stimulate osteoclasts to pull calcium from the bones. This may be detrimental in a patient with reduced bone density.

MANAGEMENT: Given the narrow therapeutic index of vitamin D and vitamin D analogs, the amounts of calcium, phosphorus, and vitamin D present in the patient's diet may need to be taken into consideration. Specific dietary guidance should be discussed with the patient and regular lab work should be monitored as indicated. Calcium, phosphorus, and vitamin D levels should be kept within the desired ranges, which may differ depending on the patient's condition. Patients should also be counseled on the signs and symptoms of hypervitaminosis D, hypercalcemia, and/or hyperphosphatemia.

boswellia food

Systemic exposure of boswellic acid extracts from boswellia serrata are variable and may be affected by administration with food. The mechanism for the interaction with food has not been fully established. One study of 12 subjects demonstrated that 786 mg of oral Boswellia serrata gum resin extract administered with a high-fat meal resulted in a prolonged AUC and Cmax of acetyl-11-keto-beta-boswellic acid (AKBA) (AUC 414%; Cmax 380%) and 11-keto-?-boswellic acid (KBA) (AUC 272%; Cmax 171%) compared to dose administration in a fasted state. In a different study, 800 mg of oral Boswellia serrata gum resin extract administered to 12 subjects exhibited a 1.4-fold increase in KBA AUC when administered with food compared to administration in a fasted state, without a difference in Cmax. No difference in AKBA pharmacokinetics was observed. The clinical significance of the administration of boswellia with food is unknown.
