Moderate drug interactions

Moderate:

fosnetupitant talazoparib

Applies to: Akynzeo for Injection (fosnetupitant / palonosetron) and talazoparib

Fosnetupitant may increase the blood levels of talazoparib in some patients. This may increase the risk of side effects such as nausea, vomiting, diarrhea, and impaired bone marrow function resulting in low numbers of different types of blood cells. You may also be more likely to develop anemia, bleeding problems, or infections due to low blood cell counts. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. You should contact your doctor if you develop paleness, fatigue, dizziness, fainting, unusual bruising or bleeding, fever, chills, diarrhea, sore throat, muscle aches, shortness of breath, blood in phlegm, weight loss, red or inflamed skin, body sores, and pain or burning during urination. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Food/lifestyle interactions

No alcohol/food interactions were found. However, this does not necessarily mean no interactions exist. Always consult your healthcare provider.

Disease interactions

7 potential interactions Applies to: 3 conditions, including Liver Disease, Immunodeficiency, and Renal Dysfunction.

Immunodeficiency

Moderate:

talazoparib

Talazoparib has a moderate potential interaction with Immunodeficiency.

Myelosuppression consisting of anemia, leukopenia/neutropenia, and/or thrombocytopenia, have been reported in patients treated with talazoparib. Therapy with talazoparib should not be started in myelosuppressed patients and patients that have not adequately recovered from hematological toxicity caused by previous therapies. If myelosuppression occurs during therapy, dose modifications are recommended. Patients should be closely monitored.

Liver Disease

Moderate:

Akynzeo for Injection

The palonosetron in Akynzeo for Injection has a moderate potential interaction with Liver Disease.

No dosage adjustment for palonosetron is necessary for patients with mild to moderate hepatic impairment (Child-Pugh score 5 to 8). Limited data are available in patients with severe hepatic impairment (Child-Pugh score greater than 9). It is recommended to avoid use of palonosetron in patients with severe hepatic impairment.

Moderate:

Akynzeo for Injection

The fosnetupitant in Akynzeo for Injection has a moderate potential interaction with Liver Disease.

Limited data are available with fosnetupitant (only available in combination with palonosetron in the US) in patients with severe hepatic impairment (Child-Pugh score greater than 9), therefore, its use should be avoided in these patients. No dosage adjustment is necessary for patients with mild to moderate hepatic impairment (Child-Pugh score 5 to 8).

Moderate:

talazoparib

Talazoparib has a moderate potential interaction with Liver Disease.

Talazoparib has not been studied in patients with moderate hepatic impairment (total bilirubin >1.5 to 3.0 × upper limit of normal [ULN] and any aspartate aminotransferase [AST]) or severe hepatic impairment (total bilirubin >3.0 × ULN and any AST). No dose adjustment is required for patients with mild hepatic impairment (total bilirubin =1 × ULN and AST > ULN, or total bilirubin >1.0 to 1.5 × ULN and any AST).

Renal Dysfunction

Moderate:

Akynzeo for Injection

The palonosetron in Akynzeo for Injection has a moderate potential interaction with Renal Dysfunction.

Mild to moderate renal impairment does not significantly affect palonosetron pharmacokinetic parameters. However, total systemic exposure increased approximately 28% in patients with severe renal impairment. Additionally, the pharmacokinetics of palonosetron have not been studied in patients with end-stage renal disease. Caution is advised.

Moderate:

Akynzeo for Injection

The fosnetupitant in Akynzeo for Injection has a moderate potential interaction with Renal Dysfunction.

No dosage adjustment for netupitant/fosnetupitant (only available in combination with palonosetron in the US), is necessary in patients with mild to moderate renal impairment (creatinine clearance of 30 to 60 mL/min). The pharmacokinetics and safety of netupitant/fosnetupitant have not been studied in patients with severe renal impairment. Severe renal impairment (creatinine clearance < 30 mL/min) did not substantially affect pharmacokinetics of palonosetron. The pharmacokinetics for netupitant and palonosetron were not studied in patients with end-stage renal disease requiring hemodialysis, therefore, avoid the use of this drug in patients with severe renal impairment or end-stage renal disease.

Moderate:

talazoparib

Talazoparib has a moderate potential interaction with Renal Dysfunction.

Talazoparib has not been studied in patients with severe renal impairment (CrCl < 30 mL/min) or patients requiring hemodialysis. Caution is advised if used in these patients. A dose reduction is recommended in patients with moderate renal impairment (CrCl 30 - 59 mL/min), and no dose adjustment is required for patients with mild renal impairment (CrCl 60 - 89 mL/min).

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
