Drug interactions

Moderate drug interactions

Moderate:

palonosetron degarelix

Applies to: Akynzeo for Injection (fosnetupitant / palonosetron) and degarelix

Using degarelix together with palonosetron can increase the risk of an irregular heart rhythm that may be serious and potentially life-threatening, although it is a relatively rare side effect. You may be more susceptible if you have a heart condition called congenital long QT syndrome, other cardiac diseases, conduction abnormalities, or electrolyte disturbances (for example, magnesium or potassium loss due to severe or prolonged diarrhea or vomiting). Talk to your doctor if you have any questions or concerns. Your doctor may already be aware of the risks, but has determined that this is the best course of treatment for you and has taken appropriate precautions and is monitoring you closely for any potential complications. You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, shortness of breath, or heart palpitations during treatment with these medications, whether together or alone. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Food/lifestyle interactions

No alcohol/food interactions were found. However, this does not necessarily mean no interactions exist. Always consult your healthcare provider.

Disease interactions

9 potential interactions Applies to: 5 conditions, including Heart Failure, Liver Disease, Long QT Syndrome, Renal Dysfunction, and Electrolyte Abnormalities.

Electrolyte Abnormalities

Moderate:

degarelix

Degarelix has a moderate potential interaction with Electrolyte Abnormalities.

Androgen deprivation therapy may prolong the QT/QTc interval. Health care providers should consider if benefits of therapy outweigh the potential risks in patients with congenital long QT syndrome, congestive heart failure, electrolyte abnormalities, or taking other drugs that can also prolong the QT interval. Consider periodic monitoring of EKG and electrolytes.

Heart Failure

Moderate:

degarelix

Degarelix has a moderate potential interaction with Heart Failure.

Liver Disease

Moderate:

Akynzeo for Injection

The palonosetron in Akynzeo for Injection has a moderate potential interaction with Liver Disease.

No dosage adjustment for palonosetron is necessary for patients with mild to moderate hepatic impairment (Child-Pugh score 5 to 8). Limited data are available in patients with severe hepatic impairment (Child-Pugh score greater than 9). It is recommended to avoid use of palonosetron in patients with severe hepatic impairment.

Moderate:

Akynzeo for Injection

The fosnetupitant in Akynzeo for Injection has a moderate potential interaction with Liver Disease.

Limited data are available with fosnetupitant (only available in combination with palonosetron in the US) in patients with severe hepatic impairment (Child-Pugh score greater than 9), therefore, its use should be avoided in these patients. No dosage adjustment is necessary for patients with mild to moderate hepatic impairment (Child-Pugh score 5 to 8).

Moderate:

degarelix

Degarelix has a moderate potential interaction with Liver Disease.

Hepatic impairment can lower degarelix exposure. It is recommended to monitor testosterone concentrations on a monthly basis in patients with hepatic impairment until medical castration is achieved. Degarelix has not been studied in patients with severe hepatic impairment and caution is therefore warranted in this group of patients.

Long QT Syndrome

Moderate:

degarelix

Degarelix has a moderate potential interaction with Long QT Syndrome.

Renal Dysfunction

Moderate:

Akynzeo for Injection

The palonosetron in Akynzeo for Injection has a moderate potential interaction with Renal Dysfunction.

Mild to moderate renal impairment does not significantly affect palonosetron pharmacokinetic parameters. However, total systemic exposure increased approximately 28% in patients with severe renal impairment. Additionally, the pharmacokinetics of palonosetron have not been studied in patients with end-stage renal disease. Caution is advised.

Moderate:

Akynzeo for Injection

The fosnetupitant in Akynzeo for Injection has a moderate potential interaction with Renal Dysfunction.

No dosage adjustment for netupitant/fosnetupitant (only available in combination with palonosetron in the US), is necessary in patients with mild to moderate renal impairment (creatinine clearance of 30 to 60 mL/min). The pharmacokinetics and safety of netupitant/fosnetupitant have not been studied in patients with severe renal impairment. Severe renal impairment (creatinine clearance < 30 mL/min) did not substantially affect pharmacokinetics of palonosetron. The pharmacokinetics for netupitant and palonosetron were not studied in patients with end-stage renal disease requiring hemodialysis, therefore, avoid the use of this drug in patients with severe renal impairment or end-stage renal disease.

Moderate:

degarelix

Degarelix has a moderate potential interaction with Renal Dysfunction.

Data on patients with moderate or severe renal impairment is limited and therefore degarelix should be used with caution in patients with CrCl < 50 mL/min.

Drug Interaction Classification

These classifications are only a guideline. The relevance of a particular drug interaction to a specific individual is difficult to determine. Always consult your healthcare provider before starting or stopping any medication.
Major Highly clinically significant. Avoid combinations; the risk of the interaction outweighs the benefit.
Moderate Moderately clinically significant. Usually avoid combinations; use it only under special circumstances.
Minor Minimally clinically significant. Minimize risk; assess risk and consider an alternative drug, take steps to circumvent the interaction risk and/or institute a monitoring plan.
Unknown No interaction information available.
