MONITOR: Coadministration with midostaurin may increase the plasma concentrations and the risk of adverse effects of drugs that are substrates of P-glycoprotein (P-gp), breast cancer resistance protein (BCRP), and/or organic anion transporting polypeptide (OATP) 1B1 transporters. The proposed mechanism, based on in vitro data, is decreased clearance due to midostaurin-mediated inhibition of P-gp, BCRP, and/or OATP1B1 transport proteins.
MANAGEMENT: Caution is advised if midostaurin is used concomitantly with drugs that are substrates of P-gp, BCRP, and/or OATP1B1 transport proteins, particularly those with a narrow therapeutic range. Dosage adjustments as well as clinical and laboratory monitoring should be considered whenever midostaurin is added to or withdrawn from therapy with these drugs. Patients should be monitored for the development of adverse effects.

Midostaurin should be taken with food to help with its absorption. Do not consume grapefruit or grapefruit juice during treatment, as it can significantly increase the blood levels of midostaurin. You may be more likely to experience side effects such as nausea; vomiting; diarrhea; swelling; high blood sugar; heart rhythm abnormalities; and impaired bone marrow function resulting in low numbers of different types of blood cells, which can increase the risk of anemia, bleeding problems, and infections. You should seek medical attention if you develop paleness, fatigue, dizziness, fainting, unusual bruising or bleeding, fever, chills, diarrhea, sore throat, muscle aches, shortness of breath, blood in phlegm, weight loss, red or inflamed skin, body sores, or pain and burning during urination. Talk to your healthcare provider if you have any questions or concerns.
Therapeutic duplication warnings
Therapeutic duplication is the use of more than one medicine from the same drug category or therapeutic class to treat the same condition. This can be intentional in cases where drugs with similar actions are used together for demonstrated therapeutic benefit.
