ADJUST DOSE: Coadministration with capivasertib may increase the plasma concentrations and risk of toxicity of drugs that are substrates of the renal transporters organic cation transporter-2 (OCT2) and/or multidrug and toxin extrusion 1 or 2K (MATE1, MATE 2K). Transient increases in serum creatinine may be observed, potentially due to capivasertib-mediated inhibition of these renal transporters. However, based on physiologically based pharmacokinetic modeling, no clinically meaningful interaction was predicted with metformin, as systemic exposure (AUC) increased by 2% to 40%, depending on the capivasertib dosing day. Clinical data with other substrates of MATE1, MATE2K, and/or OCT2 are lacking.

MANAGEMENT: Caution is recommended with concomitant administration of capivasertib with OCT2, MATE 1, and/or MATE 2K substrates. According to the manufacturer, if coadministration is required, patients should be monitored for drug-related toxicities and dosage reductions considered as needed in accordance with the product labeling of the coadministered drugs.

Consumption of grapefruit, grapefruit juice, and supplements that contain grapefruit should be avoided during treatment with capivasertib as they may increase the blood levels of capivasertib. This may increase the risk of side effects such as high blood sugar levels, diarrhea, skin reactions, nausea, vomiting, tiredness, and changes in certain blood tests. Talk to your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.
