Lumps or Pain Within the Scrotum

See your doctor. Since the pain could be coming from the testicle, you need evaluation for possible infection (such as epididymitis or as orchitis), serious inflammation, or a cancerous growth. Seek urgent medical if the swelling is increasing, or the pain is getting worse with time.

If the painful swelling is not part of the testicle, it's rare for this to be a sign of a serious problem such as cancer.
