Reason for the Shortage

  • Triad Group has recalled all alcohol prep pads, swabs, and Swabsticks due to potential contamination with Bacillus cereus. This recall has affected several medication kits that contain the prep pads. The medications within the kits are not affected and can be used. However, the alcohol prep pads need to be discarded. Unaffected alcohol prep pads from another manufacturer or sterile gauze with isopropyl alcohol can be used as a substitute.
  • Triad is unable to provide information on which companies purchased  their alcohol prep pads therefore, this list of affected medication kits may not be complete.
  • The Triad affected pads may also be packaged under Boca/Utilet, Cardinal Health, Conzelin, CVS, Moore Medical, PSS Select, VersaPro, and Walgreens. Do not use any of these affected alcohol prep pads at this time.
  • Lovenox and Arixtra discharge kits are also affected by this shortage and patients should be instructed to discard alcohol prep pads. Unaffected alcohol prep pads from another manufacturer or sterile gauze with isopropyl alcohol can be used as a substitute.

  • Interferon beta-1b (Betaseron) 0.3 mg vials with administration kits (NDC 50419-0523-35) from Bayer.
  • Interferon beta-1b (Extavia) 0.3 mg vials (NDC 00078-0569-12) with administration kits from Novartis.
  • Methylnaltrexone (Relistor) Administration Kits from Salix.
  • Peginterferon alfa-2a (Pegasys) Convenience Packs (NDC 00004-0352-39) from Genentech.
  • Somatropin (Nutropin AQ) 10 mg and 20 mg Pen Cartridges from Genentech.
  • Tenecteplase (TNKase) Administration Kits (NDC 50242-0038-61) from Genentech.
  • Triptorelin (Trelstar) 3.75 mg (NDC 52544-0189-76), 11.25 mg (NDC 52544-0188-76), and 22.5 mg (NDC 52544-0092-76) Mixject Kits from Watson.

Education Kits (no medication included)

  • Sanofi-Aventis has enoxaparin (Lovenox) discharge kits on back order and the company cannot estimate a release date.
  • GlaxoSmithKline has fondaparinux  (Arixtra) discharge kits on back order and the company cannot estimate a release date.
