For Reversal of Neuromuscular Blockade "I work with anesthesiologists and certified registered nurse anesthetists CRNAs in the OR when monitoring a patient's nervous system. Surgeons will often ask for paralytic (usually long lasting NMB is used) for exposure. When the surgeon begins decompression around nerve roots of the spine or wants to test pedicle screws, it is important for technologists in my field to have the paralytic traces in the body almost gone. We have a criteria established by experts in our field, that allows us to give the surgeon accurate information based on the manipulation that is being done to the patient. Neostigmine in my experience is good to use if time is not critical. If time is critical, say patient has one twitch response from a train of four and surgeon wants to test a pedicle screw within a few minutes, sugammadex works best. Technologist can get 4/4 twitches quickly and surgeon can test the screws without any real delay."
