MONITOR: The coadministration of nesiritide with intravenous vasodilators such as nitroglycerin, nitroprusside, and milrinone has not been evaluated. Theoretically, an increase in symptomatic hypotension may occur due to possible additive pharmacodynamic effects in the vasculature. A similar risk may exist during coadministration with oral vasodilators such as hydralazine and minoxidil and other agents with vasodilating effects.

MANAGEMENT: Whether used alone or with other agents, nesiritide should be administered only in settings where blood pressure can be closely monitored. The dosage should be reduced or the drug discontinued in patients who develop hypotension.
