Usual Adult Dose for Erectile Dysfunction

Erectile dysfunction:
10 mg orally taken prior to anticipated sexual activity. The maximum recommended dosing frequency is once per day in most patients.
Alternatively, 2.5 mg orally once daily, without regard to timing of sexual activity. May increase to 5 mg orally once daily based on efficacy and tolerability.

Erectile dysfunction and benign prostatic hyperplasia together:
5 mg orally taken at approximately the same time every day.

Usual Adult Dose for Pulmonary Hypertension

40 mg orally once daily, with or without food. Dividing the dose (40 mg) over the course of the day is not recommended.

Usual Adult Dose for Benign Prostatic Hyperplasia

Benign prostatic hyperplasia and benign prostatic hyperplasia with erectile dysfunction:
5 mg orally taken at approximately the same time every day.

Comments: Tadalafil use with finasteride for benign prostatic hyperplasia (BPH) is recommended for up to 26 weeks. The incremental benefit of tadalafil decreases from 4 weeks until 26 weeks, and the incremental benefit of tadalafil beyond 26 weeks is unknown.

Renal Dose Adjustments

Erectile Dysfunction:
For use as needed: 5 mg not more than once in every 72 hours in patients with creatinine clearance less than 30 mL/min or end-stage renal disease on hemodialysis. For patients with creatinine clearance 30 to 50 mL/min: 5 mg not more than once per day, and the maximum dose should be limited to 10 mg not more than once in every 48 hours.
For daily use: CrCl more than 30 mL/min: no dose adjustment required. CrCl less than 30 mL/min and on hemodialysis: not recommended.

Benign Prostatic Hyperplasia and Erectile Dysfunction/Benign Prostatic Hyperplasia
Creatinine clearance 30 to 50 mL/min: A starting dose of 2.5 mg is recommended. An increase to 5 mg may be considered based on individual response.

Pulmonary Hypertension:
CrCl 51 mL/min to 80 mL/min: Start dosing at 20 mg orally once a day. Increase to 40 mg orally once daily based on individual tolerability. CrCl less than 30 mL/min and on hemodialysis: Avoid use of tadalafil.

Liver Dose Adjustments

Erectile Dysfunction:
For Use as Needed: Mild or moderate hepatic impairment (Child-Pugh Class A or B): the dose should not exceed 10 mg once daily.
For Once Daily Use: Mild or moderate (Child Pugh Class A or B): caution is advised.

Severe hepatic impairment (Child-Pugh Class C): the use of tadalafil is not recommended.

Pulmonary Hypertension:
Mild or moderate hepatic impairment (Child-Pugh Class A or B): consider a starting dose of 20 mg orally once per day.
Severe hepatic impairment (Child-Pugh Class C): the use of tadalafil is not recommended.

Dose Adjustments

Erectile Dysfunction:
The dose may be increased to 20 mg or decreased to 5 mg, based on individual efficacy and tolerability.

Precautions

Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).

Dialysis

Avoid use of tadalafil.
