Valrubicin (applies to Valstar) perforated bladder

Major Potential Hazard, Moderate plausibility. Applicable conditions: Rupture of Bladder

Valrubicin is contraindicated in patients with perforated bladder or compromised bladder mucosa, including small bladder capacity that is unable to tolerate a 75 mL instillation. Evaluate the bladder before the intravesical instillation for severe irritable bladder symptoms, and in case of bladder perforation, delay the administration until bladder integrity has been restored. If this agent is administered to patients with bladder rupture or if perforation is suspected, significant systemic exposure may occur following intravesical administration and myelosuppression is possible. Weekly monitoring of complete blood counts should be performed for 3 weeks.

Valrubicin is contraindicated in patients with current urinary tract infection.
