Osteo-Form Chewable Tablets

(Calcium-Phosphorous with Vitamins)

Suggested Use: As a supplemental source of calcium, phosphorus, and vitamins A and D in dogs and cats.

Preparation: Feed from the hand or crumble and mix into food.

Feeding Directions: Feed 1 to 2 tablets per 5 kilograms (11 pounds) of body weight daily.

Note: Two Osteo-Form chewable tablets are equivalent to one slightly rounded teaspoonful of Osteo-Form SA powder.

Osteo-Form Chewable Tablets Caution

When using Osteo-Form chewable tablets as a maintenance nutritional supplement, calcium and phosphorus from other food sources should be considered.

Guaranteed Analysis per Tablet: Minerals: Calcium min. 18.5% - max 22.2% (600 mg) and Phosphorus min. 10.0% (335 mg). Vitamins: Vitamins A, min. 750 IU and Vitamin D₃, min. 75 IU.

Ingredients: Dicalcium phosphate, brewers dried yeast, calcium carbonate, microcrystalline cellulose, lactose, glycerin, silicon dioxide, ascorbic acid (a preservative), magnesium stearate, cod liver oil, vitamin A palmitate, BHT (a preservative), vitamin D₃ supplement. Artificial colors: Yellow 6 Lake, Red 40 Lake, Blue 1 Lake.
