H2 antagonists such as cimetidine may increase plasma concentrations of some benzodiazepines and enhance sedation. The mechanism may be related to inhibition of liver cytochrome P450 enzymes responsible for metabolism of certain benzodiazepines. This interaction appears to be minimal with lorazepam, oxazepam, and temazepam. Ranitidine may increase or decrease benzodiazepine plasma levels, however, the effects are not expected to be clinically significant. This interaction has not been noted with other H2 antagonists. Clinical monitoring of patient response and tolerance is recommended and benzodiazepine dosage reductions may be indicated.
