GENERALLY AVOID: Coadministration of opioids with irreversible monoamine oxidase inhibitors (MAOIs) has been associated with isolated reports of marked sedation, hypotension, respiratory depression, and coma. The mechanism is unknown, but may be related to additive CNS and respiratory depressant effects. The interaction has been reported with propoxyphene, morphine and methadone, although the safe use of MAOIs with opioids including methadone and morphine has also been reported. Linezolid is a reversible, nonselective MAOI and may theoretically interact similarly with opioids. However, the interaction has not been studied or reported in the medical literature. Moclobemide, a reversible inhibitor of monoamine oxidase A, has been reported to potentiate the effects of propoxyphene in animals.

MANAGEMENT: The use of opioids should be avoided in patients taking MAOIs or within 14 days of stopping such treatment. The same precaution may be applicable to linezolid, although clinical data are lacking.
