Usual Adult Dose for Hypertension
Initial dose: 5 mg orally once a day.
Maintenance dose: up to 40 mg orally once a day, titrated to patient response and tolerability. Doses can be increased at 2 week intervals.
Renal Dose Adjustments
CrCl less than 30 mL/min: 2.5 mg orally once a day. Titration upward should be done cautiously.
Liver Dose Adjustments
Nebivolol is contraindicated in patients with severe liver dysfunction.
Mild to moderate liver dysfunction: 2.5 mg orally once a day. Titration upward should be done cautiously.
Precautions
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).

Nebivolol may be used alone or in combination with other antihypertensive agents.
Nebivolol may be taken without regard to meals.
