Usual Adult Dose for Congestive Heart Failure
Initial dose: 1 tablet of (hydrochlorothiazide-lisinopril 12.5 mg/10 mg) once a day. Determine dosage by individual titration.
Maintenance dose: May increase up to a maximum of 2 tablets of (hydrochlorothiazide-lisinopril 25 mg/20 mg) twice a day.

Renal Dose Adjustments
The usual regimens of therapy with hydrochlorothiazide-lisinopril need not be adjusted as long as patient creatinine clearance is 30 mL/min or greater. In patients with more severe renal impairment, loop diuretics are preferred to thiazides, so hydrochlorothiazide-lisinopril is not recommended.
Liver Dose Adjustments
Thiazide diuretics should be used with caution in patients with hepatic impairment.
Dose Adjustments
The hydrochlorothiazide dose should generally not be increased until 2 to 3 weeks have elapsed.
Precautions
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
Dialysis
Lisinopril can be removed by hemodialysis.
