Usual Adult Dose for Hypertension
Initial dose: 10 mg/day orally in 1 to 2 divided doses.
Maintenance dose: 20 to 75 mg/day orally in 2 to 4 divided doses.
Renal Dose Adjustments
CrCl < 30 mL/min: Initial dose: 5 mg orally once every other day.
CrCl 30-60 mL/min: Initial dose: 5 mg orally once a day.

Dose adjustments can be made weekly or monthly until blood pressure is controlled. With long-term therapy, some tolerance may occur and the dose may need to be increased.
