Usual Adult Dose for Hypertension
Initial dose: 1 tablet of (bisoprolol-hydrochlorothiazide 2.5 mg/6.25 mg) once a day. Determine dosage by individual titration.
Maintenance dose: May increase up to a maximum dose of 2 tablets of (bisoprolol-hydrochlorothiazide 10 mg/6.25 mg) once a day.
Renal Dose Adjustments
Use bisoprolol-hydrochlorothiazide with caution in severe renal disease. The plasma half-life of bisoprolol is increased up to threefold in patients with creatinine clearance less than 40 mL/min. Hydrochlorothiazide is generally not effective in patients with creatinine clearance less than 25 mL/min.
Liver Dose Adjustments
Use bisoprolol-hydrochlorothiazide with caution in impaired hepatic function.
Dose Adjustments
The dose may be titrated every 14 days.
Precautions
If withdrawal of bisoprolol-hydrochlorothiazide therapy is planned, it should be achieved gradually over a period of about 2 weeks.
Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
Dialysis
Since there is no indication that hydrochlorothiazide is dialyzable, and limited data suggest that bisoprolol is not dialyzable, drug replacement is not necessary in patients undergoing dialysis.
