Usual Adult Dose for Osteoporosis

One tablet orally once weekly. Tablet should only be swallowed upon rising for the day with a full glass of water (6 to 8 ounces) and patients should not lie down for at least 30 minutes and until after their first food of the day.

Renal Dose Adjustments

No dosage adjustment is recommended for patients with mild to moderate renal insufficiency (creatinine clearance 35 to 60 mL/min). Alendronate-cholecalciferol is not recommended for patients with a CrCl less than 35 mL/min.

Liver Dose Adjustments

No studies were conducted in patients with hepatic impairment, but evidence shows that alendronate is not metabolized or excreted in the bile, therefore no dosage adjustment is necessary.

Vitamin D3 may not be adequately absorbed in patients with malabsorption due to inadequate bile production.

Precautions

Due to the risk of esophageal problems, patients should be instructed not to take alendronate-cholecalciferol at bedtime or before rising for the day. Alendronate-cholecalciferol should be administered with 6 to 8 ounces of plain water, at least 30 minutes before the first food, beverage, or medication of the day. Patients should remain upright for at least 30 minutes following administration of alendronate-cholecalciferol and until after their first food of the day. Safety and effectiveness have not been established in pediatric patients (less than 18 years of age).
