Hydroflumethiazide / Reserpine Dosage

Usual Adult Dose for Hypertension
Initial dose: 1 to 2 tablets (hydroflumethiazide/reserpine 25 mg-0.125 mg) orally once a day or 1 tablet (hydroflumethiazide/reserpine 50 mg-0.125 mg) orally once a day.
Should conform to the dosages of the individual components established during titration.
Maintenance dose: May increase to 1 tablet (hydroflumethiazide/reserpine 50 mg-0.125 mg) orally twice a day if necessary.
Renal Dose Adjustments
Reserpine-hydroflumethiazide is not recommended in patients with severe renal impairment because the half-life of reserpine is significantly prolonged in patients with renal insufficiency and hydroflumethiazide is not expected to be filtered into the renal tubule (its site of action) when the glomerular filtration rate is less than 25 mL/min.
Liver Dose Adjustments
Thiazides should be used with caution in patients with impaired hepatic function or progressive liver disease, since minor alterations of fluid and electrolyte balance may precipitate hepatic coma.
Dose Adjustments
The dose may be increased as needed every 1 to 2 weeks.
Precautions
Doses higher than 0.25 mg daily of reserpine should be used cautiously, because occurrence of serious mental depression and other side effects may increase considerably.
Dialysis
Reserpine is not dialyzable.
Other Comments
Daily doses of hydroflumethiazide greater than 200 mg are not recommended.
