Usual Adult Dose for Hypertension
Initial dose: 1 tablet orally once a day (should conform to the dosages of the individual components established during titration).
Maintenance dose: 1 to 2 tablets orally once a day.
Renal Dose Adjustments
Reserpine-chlorthalidone is not recommended in patients with renal insufficiency. Chlorthalidone is not expected to be filtered into the renal tubule (its site of action) when the glomerular filtration rate is less than 10 mL/min, and the elimination half-life of reserpine may be significantly increased in patients with renal insufficiency.

Dosage adjustments may occur as needed every 1 to 2 weeks. Less than 15% of patients with hypertension require daily reserpine doses greater than 0.5 mg.

If the antihypertensive effect associated with 2 tablets once a day is inadequate, addition of another (nondiuretic) antihypertensive agent or alternative therapy is recommended.
