Usual Adult Dose for Erosive Esophagitis

For healing: 20 mg orally once a day

• Duration of therapy: 8 weeks

For maintenance of healing: 10 mg orally once a day

• Duration of therapy: Up to 6 months

Use: For healing, and to maintain healing, of all grades of erosive esophagitis and relief of heartburn associated with erosive esophagitis

Usual Adult Dose for Helicobacter pylori Infection

Dual therapy: 20 mg orally twice a day, taken concomitantly with amoxicillin

• Duration of therapy: 14 days

Triple therapy: 20 mg orally twice a day, taken concomitantly with amoxicillin and clarithromycin

• Duration of therapy: 14 days

Comments:

• The manufacturer product information should be consulted for amoxicillin and/or clarithromycin dosing.
• Administer each dose of this drug in the morning and evening, 12 hours apart.

Uses:

• In combination with amoxicillin and clarithromycin, for the treatment of Helicobacter pylori (H pylori) infection
• In combination with amoxicillin, for the treatment of H pylori infection

Renal Dose Adjustments

Erosive Esophagitis: Treatment/Healing

• Estimated GFR (eGFR): 30 mL/min or greater: 20 mg orally once a day
• eGFR less than 30 mL/min: 10 mg orally once a day

Erosive Esophagitis: Maintenance of Healing

• No dose adjustment recommended.

H pylori Infection: Treatment

• eGFR 30 mL/min or greater: 20 mg orally twice a day
• eGFR less than 30 mL/min: Use is not recommended

Comment:

• The manufacturer product information for amoxicillin and/or clarithromycin should be consulted for dose recommendations in patients with renal dysfunction.

Liver Dose Adjustments

Erosive Esophagitis: Treatment/Healing

• Mild Liver Dysfunction (Child-Pugh A): 20 mg orally once a day
• Moderate Liver Dysfunction (Child-Pugh B): 10 mg orally once a day
• Severe Liver Dysfunction (Child-Pugh C): 10 mg orally once a day

Erosive Esophagitis: Maintenance of Healing

• No dose adjustment recommended.

H pylori Infection: Treatment

• Mild Liver Dysfunction (Child-Pugh A): 20 mg orally twice a day
• Moderate Liver Dysfunction (Child-Pugh B): Use is not recommended
• Severe Liver Dysfunction (Child-Pugh C): Use is not recommended

Comment:

• The manufacturer product information for amoxicillin and/or clarithromycin should be consulted for dose recommendations in patients with liver dysfunction.

Precautions

CONTRAINDICATIONS:

• Hypersensitivity to either the active component or to any of the ingredients; reactions to this drug have included anaphylactic shock
• With rilpivirine-containing products
• For contraindications of antibacterial agents (i.e., clarithromycin and amoxicillin) indicated in combination with this drug, refer to the Contraindications section of the corresponding manufacturer product information of those agents.

Safety and efficacy have not been established in patients younger than 18 years.

Hemodialysis: Removes 0.94% of administered dose
Peritoneal dialysis: Data not available

Other Comments

Administration advice:

• This drug may be taken with or without food.
• Swallow each tablet whole; do not crush or chew.
• Missed dose:
• For the healing or maintenance of healing of erosive esophagitis: If a dose is missed, administer as soon as possible within 12 hours after the missed dose. If more than 12 hours have passed, skip the missed dose and administer the next dose at the regularly scheduled time.
• For the treatment of H pylori infection: If a dose is missed, administer as soon as possible within 4 hours after the missed dose. If more than 4 hours have passed, skip the missed dose and administer the next dose at the regularly scheduled time. Continue the normal dosing schedule until the treatment is completed.

Storage requirements:

• Store between 20C and 25C (68F and 77F); excursions permitted between 15C and 30C (59F and 86F).

Monitoring:

• Metabolic:
• Magnesium levels (at baseline and periodically during prolonged therapy; with concomitant use of digoxin or other drugs that cause hypomagnesemia)
• Calcium levels (in patients with preexisting risk of hypocalcemia, at baseline and periodically during prolonged therapy)

Patient advice:

• Read the US FDA-approved patient labeling (Patient Information).
• Inform your health care provider if you experience signs and/or symptoms associated with acute tubulointerstitial nephritis.
• Notify your health care provider immediately if you experience diarrhea that does not improve.
• Report any fractures, especially of the hip, wrist, or spine to your health care provider.-Discontinue use of this drug at first appearance of a severe cutaneous adverse reaction or other sign of hypersensitivity and notify health care provider.
• If receiving this drug long-term, report any clinical symptoms of Vitamin B12 deficiency to your health care provider.
• Contact Phathom Pharmaceuticals, Inc. if exposed to this drug during pregnancy.
• Do not breastfeed during treatment.
