MONITOR: Coadministration with tazemetostat may decrease the plasma concentrations of drugs that are primarily metabolized by the CYP450 3A4 isoenzyme. The proposed mechanism is increased clearance due to induction of CYP450 3A4 by tazemetostat. According to the product labeling, coadministration of tazemetostat (800 mg twice daily) with the sensitive CYP450 3A4 substrate midazolam in patients led to reduction in the systemic exposure (AUC 0 to 12 hours) and peak plasma concentration (Cmax) of midazolam by 40% and 21%, respectively. The interaction may be more significant for sensitive CYP450 3A4 substrates.
MANAGEMENT: Caution is advised when tazemetostat is used concomitantly with drugs that undergo metabolism by CYP450 3A4. Dose adjustments as well as clinical and laboratory monitoring may be appropriate for some drugs whenever tazemetostat is added to or withdrawn from therapy.

Do not consume grapefruit or grapefruit juice during treatment with tazemetostat. Grapefruit juice can significantly increase the blood levels and effects of tazemetostat, which may increase the risk of serious side effects such as bleeding complications, pain, bone pain, tiredness, shortness of breath, excess fluid around the lungs, and development of new (second) cancers. Talk to your doctor if you have any questions or concerns. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

If you are receiving therapy with sildenafil you should avoid the regular consumption of large amounts of grapefruits and grapefruit juice. Grapefruit can raise the levels of sildenafil in your body and delay the time it takes for the medication to work. Do not increase or decrease the amount of grapefruit products in your diet without first talking to your doctor.
