Antibiotics such as nalidixic acid may reduce the effects of ethinyl estradiol in some women. If you are using ethinyl estradiol for birth control, you may be at increased risk for pregnancy or breakthrough bleeding. Talk to your healthcare provider to determine if you should use an alternative or additional form of birth control during treatment with nalidixic acid. In addition, some birth control preparations may include "placebo" non-hormonal pills that contain iron. If you are taking these iron pills during treatment with nalidixic acid, you should separate the dosing times by several hours so that the iron does not interfere with absorption of the antibiotic. Ask your doctor or pharmacist for information on when best to take these medications. It is important to tell your doctor about all other medications you use, including vitamins and herbs. Do not stop using any medications without first talking to your doctor.

Grapefruit juice may increase the blood levels of certain medications such as norgestimate. You may want to limit your consumption of grapefruit and grapefruit juice during treatment with norgestimate. However, if you have been regularly consuming grapefruit or grapefruit juice with the medication, then it is advisable for you to talk with your doctor before changing the amounts of these products in your diet, as this may alter the effects of your medication. Contact your doctor if your condition changes or you experience increased side effects. Orange juice is not expected to interact.

Nalidixic acid and multivitamin with minerals should not be taken orally at the same time. Products that contain magnesium, aluminum, calcium, iron, and/or other minerals may interfere with the absorption of nalidixic acid into the bloodstream and reduce its effectiveness. If possible, it may be best to avoid taking multivitamin with minerals while your are being treated with nalidixic acid. Otherwise, you should take nalidixic acid either 2 hours before or 2 hours after the multivitamin with minerals dose. Talk to a healthcare professional if you are not sure whether a product contains something that could potentially interact with your medication or if you have questions on how to take this or other medications you are prescribed. Using caffeine together with nalidixic acid may increase the effects of caffeine. Contact your doctor if you experience headache, tremor, restlessness, nervousness, insomnia, and increased blood pressure or heart rate.
