CONTRAINDICATED: Class I antiarrhythmic agents and ibutilide have additive effects and may potentially prolong cardiac refractoriness.
MANAGEMENT: Class I antiarrhythmic agents should not be given concomitantly with ibutilide or within four hours after ibutilide is given. In clinical trials, other class I agents were withheld for at least five half-lives prior to ibutilide infusion and for four hours after dosing, but thereafter were allowed at the physician's discretion.

Applies to: procainamide
Ethanol may increase the acetylation of procainamide. Subtherapeutic plasma levels of procainamide may result in some patients. Because the acetylated metabolite of procainamide also possesses antiarrhythmic properties, the clinical effects are unclear.
