MONITOR: One case report suggests elevated phenytoin levels associated with concomitant tizanidine use. The mechanism is unknown, but postulated to be competitive inhibition of hepatic enzymes. Drowsiness was the only adverse effect noted, and a dosage reduction of 20% was required. A similar interaction may occur with other hydantoins.

MANAGEMENT: Until further information is available close clinical and laboratory monitoring for toxicity is recommended. Dose adjustment may be necessary if an interaction is suspected. Patients should be advised to notify their physician if they experience symptoms of possible hydantoin toxicity, including drowsiness, visual disturbances, change in mental status, seizures, nausea, or ataxia.
