Dronedarone may increase the blood levels of drospirenone in some patients. This can increase the risk of developing hyperkalemia, or high levels of potassium in the blood, which in severe cases can lead to kidney failure, muscle paralysis, irregular heart rhythm, and cardiac arrest. Hyperkalemia is usually more likely to occur in people who are dehydrated or have kidney disease, diabetes, or advanced heart failure. Regular or long-term use of nonsteroidal anti-inflammatory drugs such as ibuprofen or naproxen (Aleve), or prescription medications such as potassium supplements or certain medications used to treat high blood pressure or heart conditions, may also increase the risk. Talk to your doctor if you have any questions or concerns. Your doctor may be able to prescribe alternatives that do not interact, or you may need a dose adjustment or more frequent monitoring to safely use both medications. You should seek medical attention if you experience nausea, vomiting, weakness, confusion, tingling of the hands and feet, a weak pulse, or a slow or irregular heartbeat, as these may be symptoms of hyperkalemia. Do not consume grapefruit or grapefruit juice during treatment with dronedarone unless directed otherwise by your doctor. Grapefruit juice can increase the blood levels of dronedarone to dangerous levels. This may increase the risk of an irregular heart rhythm that may be serious and potentially life-threatening, although it is a relatively rare side effect. You may be more susceptible if you have a heart condition called congenital long QT syndrome, other cardiac diseases, conduction abnormalities, or electrolyte disturbances (for example, magnesium or potassium loss due to severe or prolonged diarrhea or vomiting). Talk to your doctor if you have any questions or concerns. You should seek immediate medical attention if you develop sudden dizziness, lightheadedness, fainting, shortness of breath, or heart palpitations during treatment.
