This is something to discuss with your wife's physician and your child's pediatrician. Often when we give drugs to pregnant or nursing mothers, since we dont have adequate studies on pregnant and nursing women, we must make a decision based upon risk vs. benefit. If the risk to the mother for not taking the drug is greater than the possible risk to the infant, the drug is often given. If the risk to the child is greater, if the drug is given, than the risk of the mother doing without, the drug is NOT given. You wife's Dr must feel that the risk to her is greater than risk to the infant (so long as he was made aware that she was breast feeding-I hope she did tell him because most Drs do not think to ask). Depression left untreated in mothers is a risky thing. If depression is serious enough, she may not be able to care for her child properly. Another thing you can and may want to consider is to cease breastfeeding. At 14 months, it is no longer essential. Babies should breast feed for at least a year, if possible. Beyond a year, it is not really necessary. It is just a pleasant time for moms and babies, to be sure but not necessary for nutrition any longer and if a potentially harmful drug needs to be given to mother, then she may want to consider weaning her child as to not expose him to the effects of the drug. Fluoxetine alters brain chemistry and this may not be a good thing for a developing brain and it has been proven that the drug is excreted in human breast milk. You and your wife may want to discuss weaning the child at this point if she is to continue the drug. Again, you may want to both go and see the Dr and discuss these matters.
