What is the meaning of (nitrate free intervals) and y we do it ?

Basically if you use nitrates 24 hours ... without a nitrate free interval then there is a risk that your body will become tolerant to it and not give the cardiac/vasodilator benefits when you need it.

Repeated administration of nitrates results in diminished relaxation of smooth muscle in blood vessels, tolerance and unwanted effects.

The main adverse effects of nitrates are a direct consequences of their main pharmacological actions and include postural hypotension and headache. Tolerance to these effects develops quite quickly but wears off after a brief nitrate free period of a few hours in each 24 hour period, making it beneficial in maintaining effectiveness of treatment.
