Well, technically it can be the size of a single malignant cell when it first starts out. I'd imagine you're asking the smallest DETECTABLE size, however, and I'd say something along the lines of pea-sized could definitely be detected, and perhaps a bit smaller if more advanced medical screening technology were used (ie not just a CT scan or MRI).
