You are on the highest dose of hydrocodone they make in the US. They do not make a long acting hydrocodone in the states either. Your next step up, as Mary mentioned, is oxycodone and I agree with her that a long acting form is the best option for chronic pain. It will hold your pain for longer periods of time so you are not on that constant rollercoaster of pain- take a pill for a couple hours relief, then wait in pain for time to take the next pill. With the long acting, you take it on the clock rather than as needed and it builds a blood level so it keeps your pain under pretty good control around the clock. If you have pain that breaks through the long acting drug, you can take a short acting drug for breakthrough. Talk to your Dr about it and see what your options are.
This is quite common. The longer you take hydrocodone, the less it works. You have become resistant to its affects.
Rather than take stronger drugs, try some other methods for the pain. Take a warm bath with epsom salts. That will relax your muscles so there is less back pain.
I had the same problem you have. Taking hydrocodone regularly I built up a resistance to it. So I cut back the pain pills and went to natural pain killers like my little hottub (2 person) and that helped. Also if you go to Dr.Oz's site www.doctoroz.com he has a lot of ideas for fighting pain. One thing you need is to build up your omega 3 in your diet. Diet makes a big difference in pain.
I'm wondering, what specifically is causing your back pain? Mine is arthritis, degenerative discs, and muscle strain.
My pain doctor finally did a radio frequency ablation on six of my back nerves and my back no longer feels like someone hit me in the back with a baseball bat.

The active ingredient in Lortab is Hydrocodone, which is the 10 part of the 10/500. The other ingredient, which is the 500 part of the 10/500 is simply acetaminophen a.k.a. Tylenol. There is now a long acting form called Zohydro Er or Hysingla ER, which also have the same active ingredient but offers more convenient dosing and around the clock pain coverage for chronic pain patients. To answer your other question, you are on the highest immediate release formulation of the Lortab. There are other immediate release forms of Hydrocodone which include Vicodin, Lorcet, and Norco. All of these have the same ingredients but different concentrations of Hydrocodone/Acetaminophen. There are none with higher concentrations of HYDROCODONE. If this strength does not help, you need to talk to your physician about more frequent dosing or a different medication that may help your condition better. I hope this helps.
