Maybe you can switch to amytriptylene. Has less side effects and MUCH less expensive. I have the same diagnosis as you do and 10-20mg. each night works for me. I also exercise, keep my weight down and eat healthy whole food diet. It's not a fun thing to have!
That seems like a low dose. You can increase it by 300mgm every 1-2 wks.
Gabapentin is a very safe med.
I have the same diagnosis and slowly got to 2100mgm with a fair response,
